The Patient Care Coordinator will be responsible for coordinating patient care services, managing patient records, and providing support to medical staff.
This position requires excellent communication skills, an ability to multitask, and the ability to work well with a diverse group of patients and medical staff.
Responsibilities:
Coordinate patient care activities and ensure the delivery of quality care.
Schedule patient appointments and coordinate with other healthcare providers.
Provide patient education on health-related topics.
Maintain accurate patient records and update them as needed.
Monitor patient progress and respond to any changes in condition.
Communicate with patients and families regarding care plans and treatments.
Ensure compliance with all relevant regulations.
