Responsible for screening and placement coordination of patients referred to both the CNH and Community Adult Day Health Care programs; Responsible for Patient Eligibility Screening; Conducts psychosocial assessments of patients in the community and monitoring treatment plans; Performs case management functions and outreach; Conducts routine visits to patients in the community; Conducts monthly patient assessments; Responsible for medication review crisis intervention; Provides supportive counseling; Responsible for coordinating patient's appointments, to include travel arrangement and the coordination of aftercare plans. Citizenship:
Citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA policy). Education:
Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. NOTE:

Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Note exception:
VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations at the GS-09 level only; must become licensed or certified at the independent, master's level within 3 years of appointment as a Social Worker. NOTE:

Social Workers must be proficient in spoken and written English in accordance with VA policy. GS-09:
In addition to the Basic Requirements above, to qualify for this position at the GS-09 level, you must meet the following requirements:
GS-09 is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level at the time of their appointment must become licensed or certified at the independent level within three (3) years of their appointment as a Social Worker. At the GS-09 level, you must fully demonstrate the knowledge, skills and abilities (KSAs) below:
Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, education and other diversified background utilizing counseling skills. Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implement a treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. Ability to implement treatment modalities in working with individual, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques; Ability to establish and maintain effective working relationship and communicate with clients, staff, and representatives of community agencies; Fundamental knowledge of medical and mental health diagnoses, disabilities and treatment procedures. The includes acute chronic and traumatic illness/injuries; common medications and their effects/side effects; and medical terminology. GS-11:
In addition to the Basic Requirements above, to qualify for this position at the GS-11 level, you must meet one of the following requirements:
Experience:
You must have at least 1 full year of post-MSW degree experience equivalent to the GS-9 grade level in the field of health care or other social work -related settings (VA or non-VA experience) AND licensure or certification in a state at the independent practice level. OR Education:
In addition to meeting the basic requirements, Successful completion of a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. At the GS-11 level. In addition to the experience or education above, you must fully demonstrate the knowledge, skills and abilities (KSAs) below:
Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic and education and other diversified backgrounds. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e., acute, chronic and traumatic illness/injuries, common medications and their effects/side effects and medical terminology) to formulate a treatment plan. Skill in independently implementing different treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The physical demands of this position involve primarily walking, talking, driving, use of the telephone, etc. Strenuous physical activity is not required although you may be expected to withstand pressure and stress as part of your day to day activities.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
